- Photo copy of ID cards with photo and signature such as
drivers license, employee ID, passports, SSS, etc.
- Marriage certificate
- Residence certificate
- Original copy of certificate
of employment, indicating length of service, position and
annual salary
- Latest ITR (Income Tax Return)
- Proof of other non-salary income
such as Certificate of Deposits, Stocks/Bonds Certificates,
Lease Contracts showing term and
monthly payments, commisions, dividends, etc.
- Photocopy of Bank Statements
or Passbooks for the last 6 months
